Dr. Hartman provides doctors slips for those who take off work, like when they go to see their medical physician, dentist, optometrist, ophthalmologist, nurse practitioner, etc.
*NOTICE: Dr. Hartman reserves the right to charge for day of or last-minute cancellations. This is on EVERY waiver and paperwork patients fill out.
All prices are subject to change. There are NO hidden fees or extra fees given for “new” patients. If prices change, her patients know a couple months in advanced BEFORE the new prices change.
NOTE: There are sensitives and allergies to smells: lotions, fragrances, perfumes, colognes etc. PLEASE do not come to the office wearing any strong smells or anything that has a scent to it or you may be asked to reschedule your appointment.
