PATIENTS: All are required to sign the below document and submit it to GEHA. If you are seeing Dr. Hartman for: Immunology, Integrative Medicine, Coaching, Scans etc.
This is a free “membership” that can be canceled at any time. It is a requirement through GEHA for all those receiving any natural, wellness, alternative, integrative etc. services. This protect both you and the practitioner you are seeing. There are laws in the U.S. that allows you to care about your own health, wellness, and care. This Lifecare Agreement explains in detail how you are protected and your practitioner. In order to see Dr. Hartman, all new patients must sign this agreement online and/or in office.
