For ALL new patients who are coming in for a 30-45mins initial consult
ALL PATIENTS: All are required to sign the below document and submit it to GEHA. If you are seeing Dr. Hartman for all services: Immunology, Integrative Medicine, Coaching, Scans etc. She is licensed through GEHA and patients are required to sign the below. If a patient has NOT signed before seeing Dr. Hartman, she has the right to fill it out for the patient on the scheduled appointment day, another day or by the end of the year. This link is on every email correspondence as well Dr. Hartman sends out once a week on Gmail (bulk email) with the information.
This is a free “membership” that can be canceled at any time. It is a requirement through GEHA for all those receiving any natural, wellness, alternative, integrative etc. services. This protect both you and the practitioner you are seeing. There are laws in the U.S. that allows you to care about your own health, wellness, and care. This Lifecare Agreement explains in detail how you are protected and your practitioner. In order to see Dr. Hartman, all new patients must sign this agreement online and/or in office.
