ALL PATIENTS: All are required to sign the below document and submit it to GEHA. If you are seeing Dr. Hartman for all services: Immunology, Integrative Medicine, Coaching, Scans etc. She is licensed through GEHA and patients are required to sign the below. If a patient has NOT signed before seeing Dr. Hartman has the legal right to fill it out for you due to you being my patient. This is on every email she sends weekly. To sign the click on: “Lifecare Agreement,” Click on “GEHA LifeCare Agreement Form” on the right side of page -it’s in RED. This is for your and Dr. Hartman’s protection. There are several laws that protect your rights to how you want your healthcare, i.e. Patients Bill of Rights, AMA Ethic Codes etc. It is a requirement through GEHA for all those receiving any natural, wellness, alternative, integrative etc. services. This Lifecare Agreement explains in detail how you are protected and your practitioner. This is a free “membership” that can be canceled at any time, which it is truly just resources for you, the laws that protect your rights to how you want your healthcare, biblical and 1st & 14th constitutional rights to what you do with your body, health, etc.  Please go to the link and fill it out before your next appointment.  Thank you!!
